Counties Manukau Health

With over 6,500 staff who fund and deliver health services to a population of over 500,000 people, Counties Manukau Health Counties Manukau District Health Board) is embarking on significant systems change towards a more local and integrated patient experience. LCPMap This is the Counties Manukau Health System Integration Programme. It involves the creation of four Locality Clinical Partnerships (LCPs) in the district as the hub of primary and community healthcare, and as key networks between primary care and hospital clinicians. This system integration programme has the potential to make significant positive impact on health outcomes and improved patient experience.

Action and Impact

We're helping the senior leadership team wade through some of the complexity and identify pragmatic steps to move this programme forward. We're advising and supporting work on service redesign, organisational change, governance, and communications. We are leading the communications and engagement work with our partners at Curative.

Together with Counties Manukau Health staff, we are telling the system integration story from a patient and clinical perspective. We've developed effective communications products including videos, a road map, a metaphorical patient profile and presentations. The video above is an example of this work, and introduces the Counties Manukau System Integration Programme. The video below tells the system integration story from a patient perspective. The profile of Saepele and his family was developed using data from avoidable admissions at Middlemore Hospital.
