Long Term Conditions Social Marketing Design

innovate change is working with Counties Manukau Health (CM Health) and East Health Trust PHO (East Health) on a social marketing programme to improve self-management by people living with long term conditions in Counties Manukau.

This social marketing programme connects with the Manaaki Hauora – Supporting Wellness campaign , which aims to provide self-management support for 50,000 people living with long term conditions across Counties Manukau by 1 December 2016. Enabling people living with long term conditions to take better care of their health helps them to get on with their lives out of hospital, improving their wellbeing and confidence.

Action

We are leading a social innovation process based on our innovative action model . We are starting by reviewing what we already know about behaviour change related to improving self-management of long term conditions. Our co-design group will then focus on what the behavioural change goals are and who will make those changes. They will be informed by ethnographic research that we will carry out with the selected target audience/s.

Through a process of ideation and analysis, we will work with the co-design group and a range of other community participants, experts and creative provocatNZDs to refine the ideas and distil the outputs.

Impact

The outcome of this work will be a strong, clear social marketing plan to improve self-management of long term conditions in the Counties Manukau community. The plan will result from a participatory and creative process, aimed at using what and who we know, the best behavioural change evidence available, and adding in fresh ideas.
